As part of the grant reporting requirements, groups that were granted life jackets or a loaner stand building kit in 2025 are required to submit at least one photo for each loaner stand location that uses yellow Sea Tow Foundation^TM grant-funded life jackets.

Please submit a separate form for each loaner station that your group manages that use the grant-funded life jackets.
